For anyone looking for therapy or counseling · Licensed in 20 U.S. states
Yes, plans that follow ACA rules pay toward therapy, but how much you pay depends on the plan. Marketplace plans have to cover counseling, psychotherapy, and treatment for substance use, and they can't put yearly or lifetime dollar limits on those benefits. The network, the deductible, and whether your therapist takes the plan decide what a session really costs.

What plans must cover
Mental health and substance use care is one of the ten essential health benefits. Every Marketplace plan includes:
Behavioral health treatment, such as psychotherapy and counseling, is covered. The plan's summary shows whether a visit has a copay, coinsurance, or counts toward the deductible.
If you need a hospital stay for mental or behavioral health, it's covered as part of the same benefit, subject to the plan's rules such as prior approval.
Treatment for substance use disorder is covered too. And conditions you had before enrolling, including mental health ones, are covered from the day the plan starts.

Parity rules
These parity points follow HealthCare.gov's mental health coverage page for Marketplace plans, read October 28, 2026. Specific benefits still depend on your state and plan.

Working with Paul
Daniels Health Advisors is the independent brokerage Paul Daniels founded, licensed in 20 U.S. states. If therapy or psychiatric care is the reason you're shopping, we begin with your providers. We search your therapist and psychiatrist in each plan's network, check your medications against the drug list, and compare what a weekly session would cost before and after the deductible. Some private plans ask health questions, and approval is not guaranteed. Some may not cover care for health problems you had before you enrolled.
Bring the names of your therapist, psychiatrist, or clinic, how often you go, and any medications you take. You don't need to share a diagnosis for us to compare plans.

“Paul was very helpful in my search for business group insurance. He was able to explain my options and answer all the questions I had before signing up. Highly recommend working with him!”
Before the first session
The plan type matters for therapy. An HMO usually won't pay for out-of-network care except in an emergency, and an EPO covers only in-network providers. A PPO lets you see providers outside the network at a higher cost. Short-term plans aren't held to ACA rules and may leave mental health care out. If you take advance tax credits and your income ends up higher than estimated, you may owe some back. Under current law, starting with 2026 taxes, there is no cap on that payback. A checklist for
Therapy coverage questions
Plans that follow ACA rules must cover behavioral health treatment, including psychotherapy and counseling. You may still owe a copay, coinsurance, or the deductible, and the therapist usually needs to be in network for the best price.
Not by a Marketplace plan. Those plans can't turn you down or charge you more for any pre-existing condition, including mental health and substance use conditions, and they cover treatment from the day coverage starts.
Marketplace plans can't put yearly or lifetime dollar limits on mental health benefits. Any visit limits or approval rules can't be stricter than the ones the plan uses for medical care.
It depends on the plan type. HMOs generally don't cover out-of-network care except emergencies, and EPOs cover only in-network providers. A PPO may pay part of the cost, and you pay more than you would in network.
They may not. Federal rules don't treat short-term plans as individual health insurance, so ACA benefit rules don't apply. Read the plan's exclusions before you count on it for therapy.
Not for every plan. A private plan (not from the Marketplace) can often be bought any time of year. Some private plans ask health questions, and approval is not guaranteed. Some may not cover care for health problems you had before you enrolled. Marketplace plans, and other plans that follow ACA rules, work differently. As of October 28, 2026, under current law, open enrollment for 2027 plans on HealthCare.gov runs from November 1, 2026, to January 15, 2027. To start coverage January 1, pick a plan by December 15. States that run their own Marketplace may set different dates, so check with your state's Marketplace. A federal appeal about these rules is pending (City of Columbus v. Kennedy, 4th Cir. No. 26-1938), with oral argument set for October 30, 2026, so confirm the dates before you enroll. Outside open enrollment, you need a special enrollment period, such as one after losing job-based coverage.
